ZOOM Health NZ provides a falls prevention medicines review service. Our New Zealand trained and New Zealand registered clinical pharmacists review your medicines and prepare a personalized report that you can share with your own health professionals.
This privacy policy explains how we collect, use, protect, and delete your information when you use this service.
2. The information we collect
When you use our falls prevention medicines review, we collect:
Contact information
Your name, email address, phone number, and country or state.
Health and medicines information
- Your age and relevant medical conditions
- Your history of falls or near falls, and related symptoms such as dizziness or confusion
- A list of your medicines using the RXNorm medicine list, and any photos of your medicine labels that you choose to upload
Payment information
Payment card details are processed securely by our payment provider. We receive confirmation of payment, but we do not see or store your full card number.
Technical information
Basic technical data, such as your device type, browser, and IP address, which helps us keep the service secure and improve performance.
We collect this information directly from you through the form and any follow up communication.

- After that, we automatically delete or de-identify your personal information, unless we are legally required to keep specific records for a longer period.
- We may retain anonymized or aggregated information that no longer identifies you, for service improvement, safety monitoring, and statistical purposes.
If you ask us to delete your information sooner, we will do so where possible and where this is consistent with legal and clinical safety obligations.

With our clinical pharmacists and essential support staff who need the information to provide your review and run the service.

You control if, when, and how you share your report with your doctor, nurse, specialist, or family. We do not send your report to your health professionals unless you ask us to.

We do not sell your data and we do not share your information for unrelated advertising.
8. International transfers
Our service is provided by ZOOM Health NZ, which is based in New Zealand. If you use this service from the United States or another country, your information will be transferred to and stored in New Zealand, and possibly in other countries where our secure service providers operate.
